Verification Forms
Your award letter will indicate all documents required to complete your financial aid file. Forms are available to download, print and sign using Adobe Acrobat Reader 4.0. You may also view your list of requirements online via BannerWeb and will receive weekly email notifications of missing or incomplete items.

Request Tax Information
Request Tax Return Transcript
When your FAFSA is selected for verification, you may be required to provide your tax information from a previous tax year. You may provide a signed copy of your Federal Tax Return, or you may request a copy of your Tax Return Transcript from the IRS online or by paper.
Online Request
Request the IRS Tax Return Transcript online.
Note: This is typically not available if you have never filed taxes before in prior years. If this is the case, please use the paper request process detailed below.
- Click “ Get Transcript Online”. If you are unable validate your identity (e.g., you cannot provide financial verification information), you may use Get Transcript by Mail option detailed below.
- Follow instructions to create an account or log in. To create an account, you will need your Social Security Number or Individual Tax Identification Number, email address, birth date, mailing address from latest tax return, tax filing status, account number for loan or credit card associated with your name, and mobile phone associated with your name. If you do not have this information, you may submit the paper request form detailed below.
- Select “ Tax Return Transcript” and enter the tax year requested by the Office of Financial Assistance (e.g. 2019) in the Tax Year field.
- If successfully validated, you will be able to view your IRS Tax Return Transcript.
- Securely upload the IRS Tax Return Transcript to the Office of Financial Assistance, if requested.
Paper Request Form – IRS Form 4506-T

- Form 4506-T - 2021 tax year (PDF)

- On the first page, complete items 1-4, following the instructions on page 2 of Form 4506-T.
- The tax-filer(s) must sign and date the form and enter their telephone number.
- Check the box in the signature area to acknowledge you have the authority to sign and request the information. The form will not be processed and returned to you if the box is unchecked.
- Mail or fax the completed IRS Form 4506-T using the information on page 2 of Form 4506-T.
- If the 4506-T information is successfully validated, tax filers can expect to receive a paper IRS Tax Return Transcript at the address provided on their request within 5 to 10 days.
- Securely upload the IRS Tax Return Transcript to the Office of Financial Assistance, if requested.
Request Verification of Non-Filing
An IRS Verification of Non-filing Letter provides proof that the IRS has no record of a filed Form 1040 for the requested year. Non-tax filers can request an IRS Verification of Non-filing Letter online or by paper.
