Reporting Instructions

This page houses the reporting forms for the University of the Incarnate Word. Each of the links below will direct you to a specific form for completion, which will then route to the appropriate department for handling. All forms should be filled out with complete, accurate, and specific information. Completing a report with incomplete or inaccurate information may impair, deter, or delay the University’s ability to take appropriate action.

This form is not meant to be an exhaustive list of all reporting mechanisms at the University, but is meant for those reports that require timely, appropriately, or mandated responses from UIW personnel to maintain the health and safety of the University.

Below are the various reporting mechanisms by topic at the University of the Incarnate Word. Below you will find instructions on reporting an incident by topic. Should you have questions regarding how to file a report, need further assistance, or are unsure which topic applies to you, students may reach out to the Office of Student Advocacy and Accountability at (210) 829-3817, and employees and other members of the UIW community can reach out to the Office of Environmental Health, Safety, and Risk Management at (210) 829-6035. Health, Safety, and Campus Security

Police reports cannot be completed online. To initiate a police report, contact UIWPD directly at (210) 829-6030 , or visit UIWPD’s headquarters, located in the lobby of Clement Hall on UIW’s Broadway campus.

If you are located at off-campus and need to file a report, call 911. Additional information regarding the University of the Incarnate Word Police Department, including other contact information and mailing addresses, is located online here: UIWPD.

CARE or Concern for a UIW Community Member. This form should be used to report behaviors, concerns, or actions observed that may pose a risk to a student’s wellbeing, or a risk of harm or danger to self or others. This includes, but it not limited to: mental health concerns or issues; significant changes in behavior; financial, food, or housing insecurity; suicidal ideation or attempt; death of a student; or other significant factors in an individual’s life.

If you suspect a community member is having suicidal ideation or attempt or poses a risk to him or herself or other members of the community, please contact UIWPD at (210) 829-6030 or call 911 before filling out this form.

Title IX Incident Report. The Office of Title IX is committing to establishing a safe and supportive educational, work, and living environmental for all members of the University committee, free from discrimination or harassment based on sex. This form should be utilized to report an incident of sexual misconduct, which includes sexual harassment, sexual assault, sexual exploitation, stalking, and relationship violence (including dating and domestic violence).

Report an Injury or Illness. This form should be utilized if you are a UIW student, employee, or community member that has sustained an injury or illness while on UIW’s campus, or while participating in a university activity.

If you have been injured on campus or require immediate or emergency response, contact UIWPD at (210) 829-6030. It is recommended that you file a police report with UIWPD first, and then submit the above-referenced report.

Employee Report of Injury Form. This form should be utilized if you have sustained a workplace injury as defined by the Texas Workers’ Compensation laws and need to make a claim for workers’ compensation. Note that this form cannot be submitted online and should be filled out and submitted to your direct supervisor and the Office of Human Resources. More information regarding Workers’ Compensation is located on the Office of Human Resources’ website, here: Workers’ Compensation. Be aware that report must be made within twenty-four (24) hours of the incident. Your supervisor will also be required to submit an Incident Report, available online: Workplace Injury/Incident Report (Supervisor).

Report an Injury (Damage). This form should be utilized if you are a UIW student, employee, or community member that has sustained damage or loss to property or automobile while on UIW’s campus, or while participating in a university activity.

If you have been involved an automobile accident or require immediate or emergency response, contact UIWPD at (210) 829-6030. It is recommended that you file a police report with UIWPD first, and then submit the above-referenced report.

Facility Work Orders. To submit a request for building maintenance and grounds-related work orders, go to the Facilities Work Orders app located in Cardinal Apps. If you need assistance in submitting a work order, please call (210) 283-5010.

This provision is open to all members of the University community and can be used to report a safety concern on campus, or a situation that could have resulted in injury or damage. To report a concern, please call (210) 283-5010. This is limited to safety concerns related to facility and premises issues only. For all other safety concerns or situations required immediate or emergency response, please contact UIWPD at (210) 829-6030.

If you suspect or know of a chemical or biological spill, you should immediately report to UIWPD at (210) 829-6030, and then to the Office of Environmental Health, Safety, and Risk Management at (210) 829-6035.

Report a Complaint about UIWPD. This form should be utilized if you are making a complaint against or have a concern with UIWPD. This form is limited to complaints regarding law enforcement or agency activity. For more information related to UIWPD’s complaint process, see UIWPD’s Complaint Website.

Academic Support and Concerns (Students Only)

If you suspect a violation of the academic integrity policy has occurred, students should report directly to the faculty or staff in charge of that course or department where the violation is occurring. Once reported, the applicable faculty or staff member will file a complaint in accordance with the procedures set forth below.

Note that professional programs may have their own policies related to academic integrity and professional misconduct, and separate grievance processes in accordance with the various accrediting bodies. If a report is made to the Office of Student Advocacy and Accountability for one of these programs, OSAA will forward to the appropriate department for handling. Faculty, staff, administration and students of professional programs should consult their applicable handbooks first before filing a complaint.

UIW Student Misconduct Report. This form should be used to report behavior(s) by individual students or student organizations that may be in violation of the UIW Student Handbook, the Student Code of Conduct, or other UIW policies. This form is directed to the Office of Student Advocacy and Accountability for handling.

Note that professional programs may have their own policies related to academic integrity and professional misconduct, and separate grievance processes in accordance with the various accrediting bodies. If a report is made to the Office of Student Advocacy and Accountability for one of these programs, OSAA will forward to the appropriate department for handling. Faculty, staff, administration and students of professional programs should consult their applicable handbooks first before filing a complaint.

Residence Life Incident Report. This form should be used to report behavior(s) by individual students that may be in violation of the UIW Residence Life Handbook, the UIW Student Handbook, the Student Code of Conduct, or other behavior occurring in one of

Cardinal Complaint Form. This form should be used to report a complaint or issue related to an administrative or academic department. Once submitted, this form is reviewed by the Dean of Campus Life or Provost’s Office, as appropriate, and then directed to the appropriate personnel for handling. Students are encouraged to reach out to the department directly to resolve their complaints prior to submitting this form.

Professional School Incident Report. This form is exclusively for general concerns or violations or University policy at the Feik School of Pharmacy, School of Rehabilitation Sciences, Rosenburg School of Optometry, and the School of Osteopathic Medicine.

Note that professional schools and programs may have their own policies and separate grievance processes in accordance with the various accrediting bodies. Faculty, staff, administration and students of professional programs should consult their applicable handbooks first before filing a report.

Reporting for Faculty, Staff, and Administration (Employees)

Report Academic Misconduct. This form should be used by faculty or staff only to report violations of the UIW Academic Integrity Policy. Note that professional programs may have their own policies related to academic integrity and professional misconduct, and separate grievance processes in accordance with the various accrediting bodies. If a report is made to the Office of Student Advocacy and Accountability for one of these programs, OSAA will forward to the appropriate department for handling. Faculty, staff, administration and students of professional programs should consult their applicable handbooks first before filing a complaint.

Title IX Incident Report. This form should be utilized if an employee suspects a student or community member has been a victim of or committed sexual misconduct, including sex and gender-based discrimination, sexual harassment, sexual assault, relationship violence (including dating and domestic violence), or stalking. Employees at the University of the Incarnate Word are mandatory reporters, which means employees are required to report potential or suspected violations of Title IX or UIW’s Sexual Misconduct Policy to the Director of Title IX.

Employees are reminded that they should not investigate into matters on their own and should immediately report a suspicion of sexual misconduct. If an employee is concerned for the welfare of a community member or believes there to be an imminent or serious risk to a community member, the employee should contact UIWPD (or 911, as applicable), first, and then submit the Title IX report.

Faculty who have concerns regarding the academic performance or progress of a student should refer to their program’s handbook, and utilize the Navigate application located in Cardinal Apps.

Campus Security Authority Reporting Form. If you are a Clery defined Campus Security Authority and need to report an incident, this form should be filled out and submitted to pdadmin@uiwtx.edu.
